Gusev has led to 20 percent less spam for pharmaceutical drugs." width="120" height="118" /></a>The New York Times reports that worldwide drug-related spam has fallen by as many as 50 billion messages per day thanks to a crackdown on infamous Russian spammer Igor A. Gusev. The huge 20 percent drop off may have gone essentially unnoticed by most people because of the widespread use of spam filters.</p>
<p>Companies that monitor the Internet, both in the U.S. and abroad, noted they saw a substantial decrease in spam beginning last week.  Antivirus company Kaspersky Lab, based in Moscow, noted that the percentage of mass emails (spam) in the United States that contained drug-related advertising was 65 percent near the end of October. That number has fallen to 41 percent in the beginning of November, the company said.</p>
<p>Gusev was reportedly heavily involved in the operations of SpamIt.com, a website believed to be a crux in the proliferation of drug-related spam. In fact, a nonprofit organization named Spamhaus, which monitors global spam, listed SpamIt.com and its partners/affiliates as the world’s largest sponsor of spam.</p>

Gusev." width="150" height="150" /></a>According to United Press International (UPI), Gusev also utilized a partner website called Glavmed.com to propagate spam. Gusev’s lawyer said he was not involved with sending email spam through SpamIt.com or any website.</p>
<p>However, SpamIt.com ceased operations on Sept. 27, six days after the Russian investigation into Gusev began. The drop in worldwide spam coincided directly with the website’s closure, according to monitoring agencies. Gusev is apparently not in Russia at this time. Russian authorities are seeking to speak with him.</p>
<p>Although no formal charges have been filed against Gusev, the investigation continues, and Russian authorities have removed several hard drives, flash cards and laptops from his place of residence. The New York Times reports that specific, computer crime-related charges may follow. <p>It seems that American Internet security experts are calling the Russian crackdown on Gusev and his partners an important, groundbreaking development that reverses the country’s trend of turning its back on cyber crime. Some have even accused the Russian government of being in some way partnered with Russian spammers, perhaps because they were profiting from the mass emails, or were utilizing the tech savvy of the spammers for other reasons.</p>
<p>However, Russian President Dmitri A. Medvedev has sought to expand and legitimize the Russian Internet industry. Earlier this year, he and his cabinet visited California to meet with Silicon Valley executives. Perhaps it is no coincidence that SpamIt.com ceased operations about two weeks before California Governor Arnold Schwarzenegger and his delegation reciprocate their visit to Moscow on Oct. 10.</p>

<p>The predominately male sex hormone testosterone is 20 times more prevalent in men than women, giving the widely believed notion that men want sex all the time. But one day, people may voluntary ask to consume toxin from the venom of the deadly Brazilian wandering spider.</p>
<p>It turns out that, when bitten by the wandering spider, some men suffer painful erections that last more than 4 hours and sometimes lead to permanent damage to the penis. This led researchers to isolate the Tx2-6 toxin in the spider&#8217;s venom, and inject it into rats to measure potential improvements in erectile function.</p>
<p>Doctors at the Medical College of Georgia in Augusta radioactively labeled and injected a purified form of the toxin into rats that suffered from high blood pressure and severe erectile dysfunction. The researchers found that the toxin had a <a href="http://www.cbc.ca/cp/HealthScout/090926/6092606AU.html ">very positive reaction</a> in the rats. Results showed improved levels of nitric oxide, which led to penile relaxation and erections.</p>
<p><a href="http://www.accessrx.com/viagra// ">Viagra</a>, <a href="http://www.accessrx.com/viagra// ">Cialis</a> and <a href="http://www.accessrx.com/viagra// ">Levitra</a> all work similarly to the isolated toxin, in that they all enhance the effects of nitric oxide, a chemical that relaxes smooth muscles in the penis during sexual stimulation and allows increased blood flow.</p>
<p>&#8220;The concept that a venom can have an effect on erection is highly plausible,&#8221; said Dr. Arnold Melman, chief of urology at Montefiore Medical Center in New York City. &#8220;This is very interesting and exciting, but also very early.&#8221;</p>
<p>Various doctors lauded the work.</p>
<p>&#8220;We do need more research,&#8221; added lead researcher Kenia Pedrosa Nunes, Ph.D., Medical College of Georgia. &#8220;I&#8217;m sure it can be a pharmacological tool that may one day be able to help patients who cannot take Viagra.&#8221;</p>
<p><strong>Topical ED Ointment Shows Promise</strong></p>
<p>Many have tried, but all have failed. Apparently, it&#8217;s pretty hard to formulate and perfect a topical ointment for erectile dysfunction.</p>
<p>Researchers at Yeshiva University in New York, however, may be on to something. They are testing a drug-delivery system that consists of nanoparticles – each smaller than a grain of pollen – in mice. And, based on the positive results, researchers will be performing further testing on the ED ointment.<a href="http://www.accessrx.com/blog/wp-content/uploads/2009/10/testing.png"><img class="alignright size-full wp-image-4499" title="testing" src="http://www.accessrx.com/blog/wp-content/uploads/2009/10/testing.png" alt="" width="273" height="177" /></a></p>
<p>If it comes to market, this topical Viagra should prove to be a safer and more effective ED therapy with less side effects. Who knows, depending on how the FDA views the safety of the product, and whether or not they want the ointment to be in the hands of the general public, the product could potentially be sold over the counter.</p>
<p>The <a href="http://www.sciencedaily.com/releases/2009/09/090918181456.htm ">groundbreaking research</a> tested nanoparticles encapsulating nitric oxide to see if they can carry tiny payloads of various drugs or other medically useful substances and successfully release them in a manner that is controllable and sustainable. The study found that Einstein nanoparticles, which have been shown to penetrate the skin, can allow the targeted delivery of compounds that treat ED. This essentially eliminates the systemic side effects caused by the three current, FDA-approved pill medications.</p>
<p>&#8220;Most of the animals, nearly 90 percent, showed a response to treatment with the nanoparticles,&#8221; says co-author Joel M. Friedman, M.D., Ph.D., professor of physiology &amp; biophysics and of medicine. And, response time to the nanoparticles was very quick, just a few minutes. Current ED pill medications do not begin to work to their full potential for at least 20 to 30 minutes.</p>
<p>Clinical studies on humans could begin in a few years if nanoparticle studies on animals continue to progress as expected. However, as investigators cautioned, the time from a proof-of-concept trial in animals to approved use in humans could be at least a decade.</p>
<p>Although studies of the spider venom treatment and nanoparticle treatment are still in their infancy, middle-aged men who currently do not suffer from ED complications may have a failsafe in case they ever do.</p>

<p>Many men have asked their doctors which erectile dysfunction medication works best, and which one has the least amount of side effects. Hopefully their doctors answered correctly&#8230;</p>
<p><a href="http://www.accessrx.com/blog/wp-content/uploads/2009/06/photo_finish.png"><img class="alignleft size-full wp-image-4501" title="photo_finish" src="http://www.accessrx.com/blog/wp-content/uploads/2009/06/photo_finish.png" alt="" width="252" height="303" /></a>According to a recent study by the Agency for Healthcare Research and Quality (part of the department of Health and Human Services), all three FDA-approved medications for ED work with equal effectiveness, and none of them appear to be more harmful (in terms of adverse events/side effects).</p>
<p>The reason for this may be that all three ED medications work in a similar way as <a href="http://en.wikipedia.org/wiki/PDE5_inhibitor ">PDE-5 inhibitors</a>.</p>
<p>To borrow a quote&#8230;</p>
<p>“The PDE-5 inhibitors are all effective, just not one over any of the others,” said Matthew Cantrell, assistant clinical professor at the University of Iowa College of Pharmacy and clinical pharmacy specialist at the Iowa City VA Medical Center. “This pretty much represents maintenance of the status quo in ED treatment. Product selection is still up to the prescriber and patient and specific factors such as price and formulary status.”</p>
<p>Even the prices of the three ED medications are very similar. On AccessRx, for example, 10 Viagra pills and 10 Levitra pills are $199. Ten Cialis pills are $219, but Cialis works for up to 36 hours while Viagra and Levitra work 4 to 5 hours. So price appears to be a push too. <p>You may have heard Los Angeles Dodgers slugger and superstar Manny Ramirez was suspended earlier this week by Major League Baseball.</p>
<p>As if the sport didn&#8217;t have enough cheaters, right? These boys just won&#8217;t let the steroid era end. Home run masher Mark McGuire won&#8217;t talk about it, A-Rod recently admitted he&#8217;s a cheat, and now Manny Ramirez has further tarnished a sport that was already a thick, gunky brown.</p>
<p>Apparently Ramirez will lose a hefty $7.7 million in pay to miss the 50 games that will keep him out until the July 4th weekend. (I wonder if the Dodgers and MLB will donate the money to charity&#8230; &#8230; &#8230; pockets of laughter&#8230; &#8230; &#8230;)</p>
<p><strong>Manny Has ED?<img src="/blog/files/media/image/Manny%20Viagra.jpg" alt="" width="130" height="138" align="right" hspace="20" vspace="20" /></strong></p>
<p>The one item that we found interesting was that <a href="http://sports.yahoo.com/mlb/news?slug=ys-ramirezsuspension050709&amp;prov=yhoo&amp;type=lgns ">at least one credible source</a> said that the illegal substance that was consumed by Ramirez was for erectile dysfunction. And immediately we wondered, since Manny has some time on his hands, will we see him on Sunday Night Baseball later this season &#8212; not hitting home runs at Chavez Ravine, but talking about &#8220;when the moment is right&#8221; for Viagra?</p>
<p>He probably wouldn&#8217;t fit in one of the Cialis dueling tubs, not with all of those steroid-ridden muscles, right? And Levitra seems akin to a small-market team in the world of ED medications, so our money is on Manny signing a lucrative ($7.7 million) contract with Pfizer, which manufactures <a href="http://www.accessrx.com/viagra// ">Viagra</a>. What a perfect fit. Rafael Palmeiro, a former spokesperson for Viagra, went down in flames in a similar steroid scandal when he was <a href="http://www.washingtonpost.com/wp-dyn/content/article/2005/08/01/AR2005080100739.html ">suspended in 2005</a>.</p>
<p>If anything, this latest Manny Ramirez debacle makes us realize that Viagra, erectile dysfunction and Major League Baseball may be more intertwined than it seems. <img src="/blog/files/media/image/Rafael%20Palmeiro.jpg" alt="" width="150" height="200" align="left" hspace="20" vspace="20" /></p>
<p><strong>HCG the Culprit?</strong></p>
<p>As the jokes continue to surface, it appears that the actual substance in question for Ramirez is HCG. According to the <a href="http://www.latimes.com/sports/la-sp-manny-ramirez8-2009may08,0,6324894.story">Los Angeles Times</a>, &#8220;A high-ranking sports doping authority said HCG could legitimately be prescribed for a man who does not naturally produce enough testosterone, but BALCO founder and convicted steroid dealer Victor Conte said it is often used to replenish testosterone levels at the end of a cycle of steroid treatments&#8230; HCG is one of dozens of substances prohibited under baseball&#8217;s policy.&#8221;</p>
<p>HCG is not an ingredient in Viagra, Cialis or Levitra. Although these three FDA-approved ED meds are not banned by most sports leagues and associations, studies are underway, believe it or not, to see if they should be banned. Unsurprisingly (think about it) they can help blood flow in all parts of the body, and some studies indicate these ED medications provide an <a href="http://articles.latimes.com/2006/jun/23/science/sci-viagra23 ">unfair advantage to athletes </a>who compete at high altitudes.</p>
<p>We may see Viagra, <a href="http://www.accessrx.com/viagra// ">Cialis</a> and <a href="http://www.accessrx.com/levitra/ ">Levitra</a> on the banned substance list yet. In which case Manny Ramirez would apparently be left with no options for his ED.</p>

<p>If you have taken FDA-approved erectile dysfunction (ED) medications, or have considered requesting a prescription, you may be familiar with some of the side effects included in the official patient information pamphlet.</p>
<p>A few of the most serious of these side effects deal with vision.</p>
<p>However, a recent study published in the April, 2009 edition of <em>Archives of Ophthalmology </em>revealed that when 212 men took Viagra and Cialis every day for three months, there were no reported changes in visual function, signs of retina damage or intraocular pressure. Likewise, after daily use for 6 months, no vision loss or impairment of any sort was reported in 155 men who continued to take the medications.<img src="/blog/files/media/image/RS%20Eye%20Chart.jpg" alt="" width="115" height="200" align="right" hspace="15" vspace="15" /></p>
<p><strong>2005 Labeling Change</strong></p>
<p>After Viagra and Cialis were put on the market, the FDA stated that there were a &#8220;small number&#8221; of reports of vision loss, which prompted <a href="http://www.fda.gov/bbs/topics/NEWS/2005/NEW01201.html ">a labeling change</a> on packages of Viagra, Cialis and Levitra.</p>
<p>Yet, the FDA said &#8220;it is not possible to determine whether these oral medicines for erectile dysfunction were the cause of the loss of eyesight or whether the problem is related to other factors such as high blood pressure or diabetes, or to a combination of these problems.&#8221;</p>

<p>Men who have ever had severe loss of vision, which might reflect a prior episode of non arteritic ischemic optic neuropathy (NAION), should share this information with their doctor, as such patients are at an increased risk of developing NAION again.</p>

<p>The authors of the above <em>Archives of Ophthalmology </em>study concluded that, with regard to the vision of study participants, &#8220;results indicate that there is no cumulative damage or effect of clinical significance for either 5 milligrams of <a href="http://www.accessrx.com/viagra// ">tadalafil (Cialis)</a> or 50 milligrams of <a href="http://www.accessrx.com/viagra// ">sildenafil (Viagra)</a> taken daily for six months.&#8221;</p>
<p>This is encouraging news for a very specific study carried out by professional ophthalmologists.</p>

<p>Has erectile dysfunction (ED) advertising become intrusive enough to affect our &#8220;normal&#8221; daily lives? Depends who you ask.</p>
<p><a href="http://www.consumerreports.org/health/medical-conditions-treatments/erectile-dysfunction-drug-ads/index.htm">Consumer Reports</a> states that manufacturers Pfizer (<a href="http://www.accessrx.com/viagra// ">Viagra</a>), Lilly (<a href="http://www.accessrx.com/viagra// ">Cialis</a>) and GlaxoSmithKline (<a href="http://www.accessrx.com/levitra/ ">Levitra</a>) spent more than $300 million in advertising their products in 2007 alone. Cialis ad spending was up to $152 million in 2007, more than 250 percent over 2006.</p>
<p>Considering that more than 18 million prescriptions were written for these drugs, and that combined sales eclipsed $3.7 billion, you can see why the manufacturers have casting calls for new commercials almost every day.</p>
<p>This is big money we&#8217;re talking about. But when does it go too far? Due simply to the demographics of certain TV audiences, the manufacturers focus their efforts in specific areas &#8212; especially sporting events.</p>
<p>Can parents watch a Sunday afternoon of NFL football with their kids, or sit through an evening watching &#8220;America&#8217;s Game,&#8221; without their own little sluggers asking why the guy in the commercial needs medicine?</p>
<p>You don&#8217;t exactly have to scour the internet to find media articles and blog posts calling for a reprieve from ED. Personally, I&#8217;ve gotten used to these commercials (don&#8217;t we fast-forward our DVR through them all now anyway?) and I don&#8217;t have kids. So, I may not be the best person to ask.</p>
<p>But on the other hand, many things that kids are exposed to these days (not the least of which are low-lifes at school or even many grown-up versions in the adult general public) are a lot worse.</p>
<p>Many things need to be explained to kids these days, and ED may seem like a walk in the park when it comes to drugs and other things we need to help our youth steer clear of. But, when it comes down to it, there&#8217;s always a non-profit we can turn to for a little guidance, right?</p>
<p>In this case, it&#8217;s the Pharmaceutical Research and Manufacturers of America, and their <a href="http://www.phrma.org/files/PhRMA%20Guiding%20Principles_Dec%2008_FINAL.pdf">16 Guiding Principles for Direct to Consumer Advertising</a>. These guys are the &#8220;policy setters&#8221; and &#8220;watchdogs&#8221; over their industry.</p>
<p>For instance, No. 16 of their guiding principles states: &#8220;In terms of content and placement, DTC television and print advertisements should be targeted to avoid audiences that are not age appropriate for the messages involved. In particular, DTC television and print advertisements containing content that may be inappropriate for children should be placed in programs or publications that are reasonably expected to draw an audience of approximately 90 percent adults (18 years or older).&#8221;</p>
<p>Talk about ambiguous, huh? What is considered &#8220;content that may be inappropriate for children?&#8221; Good question. It is something that <a href="http://www.theage.com.au/national/impotence-message-too-potent-so-down-it-comes-20080825-425o.html ">people all over the world debate</a>. <p>For a while there, it looked like we would never see another drug manufacturer challenge the highly competitive market for erectile dysfunction (ED) drugs. The two big dogs, Viagra and <a href="http://www.accessrx.com/cialis/">Cialis</a>, control 90 percent of the annual revenue for the $3.7 billion market. <a href="http://www.accessrx.com/levitra">Levitra</a> controls the rest.</p>
<p>Now, within a matter of weeks, two new potential ED drugs have come out of the woodwork. This blog highlighted <a href="http://www.accessrx.com/blog/mens-health/potential-new-ed-drug-zoraxel/">Rexahn&#8217;s drug named Zoraxel</a> late last month. And now, VIVUS (which also provides prescription urethral suppository Muse, as well as Actis, a latex constriction loop) has announced a second Phase 3 trial for their ED drug named Avanafil.</p>
<p>It has been more than five years since the FDA approved a drug for ED, and that was for <a href="http://www.accessrx.com/viagra// ">Cialis</a> back in November of 2003. It has taken Cialis five years, but its $1.44 billion in annual worldwide sales is now approaching Viagra&#8217;s $1.93 billion.</p>
<p>All things being equal, <a href="http://www.accessrx.com/levitra/ ">Levitra</a> is generally considered to be just as effective as Viagra and Cialis, but it seems Levitra/Bayer/Glaxo Smith-Kline are not willing to ante up to try to take a bigger slice of the market. They seem happy with their $300+ million annual return.</p>
<p><a href="http://www.accessrx.com/viagra// ">Viagra</a> has been around for more than 10 years, has the history and tradition of a grizzled veteran. Cialis lasts for up to 36 hours and is a rising star.</p>
<p>So one has to ask:  How much room is there for another new kid on the block? Sure, sheer curiosity may provide a quick buck or two, but how many of the 25+ million men who have used Viagra, most with success, will even waver? Hard to say, but the number is sure to grow when the Viagra patent for active ingredient Sildenafil Citrate expires in March, 2012.</p>
<p>At that point, generic versions are sure to find a way to market, and once they are FDA-approved, prices for ED meds are sure to drop quite a bit &#8212; which should make fun in the bedroom more economical.</p>
<p>Although Zoraxel and Avanafil are probably at least a couple years away from the market, their introduction could shake things up depending on how they are priced, and what new benefits they provide. As far as Avanafil is concerned, here is the scoop from the VIVUS press release concerning Phase 3 trials for diabetics.</p>
<p>&#8220;Early data suggest that avanafil may provide a fast-acting therapy with a short half-life, which, if approved, may represent a noteworthy advance for the millions of men living with ED and searching for new options.&#8221;</p>
<p>That&#8217;s the question, how many men are searching for new options. There are already three proven medications, and who needs one that doesn&#8217;t last as long? Success for Avanafil may depend on how quickly it works. The other ED meds can start working in about a half an hour, sometimes even sooner. <p>The success of Zoraxel may hinge on the fact it apparently may have less side effects. Every pill needs a plug, and without it, success is not a given. Just ask Levitra/Bayer. They have no gimmick, and they&#8217;ve been lapped like a race car equipped with a V-6.</p>
<p>Considering how much money it takes to get these medications to market, there will be lots of pressure for success from investors, to say the least. It will be interesting to see how these ED medications attempt to set themselves apart from the rest. And considering the competition, they might need more money for marketing than they did for research and development.</p>
